At Banzai, you can take up to 12 full weeks of maternity leave—2 weeks with 100% of your current salary and 4 weeks as short-term disability (60% pay based on last year's w2). Short-term disability is provided by Unum.

Unum will pay you in weekly payments during your 4 week maternity leave. Your total salary outlined in last year's W2 will be divided by 52. 60% of that number will be what Unum pays each week. This money is subject to taxes.
File 4 weeks prior to your expected due date (it's okay if the date of your child's birth on the claim is different than the actual birth date of your child).
Healthy Beginnings isn't exactly maternity leave, but it's a benefit provided to you through SelectHealth that gives you cash payments for going to pre and postpartum appointments. You'll also be given a direct line to a nurse to ask questions throughout your pregnancy. If your spouse is having a baby and is covered by the Banzai health plan, they can benefit from this program.
